Consent & Aftercare Form Library

Regardless of the services you offer/perform it's imperative you have every client review & sign a consent form prior to any work being done. This not only provides a further understanding of what the service will entail but also sheds light on the possible side effects and provides a release of liability for the aesthetician/artist. Over the years we've put together several consent/aftercare forms that satisfy underwriting requirements, should you wish to use your own underwriters will review your forms to determine if required wording is present. 

One of the most important sections of any consent form is the Hold Harmless Clause, for your reference we've provided sample wording below:

Hold Harmless Clause Sample: "I agree to release and forever discharge and hold harmless the (technician title i.e. tattoo artist, aesthetician, etc...) and all employees from any and all claims, damages or legal actions arising from or connected in any way with my (name specific service being provided), or the procedure and conduct used in his/her (specific service) procedure."
